What to include in your referral

• Patient name, date of birth and medicare number
• Reason for referral - presenting symptoms, and specific clinical question
• Relevant medical history - (Eg: diabetes, family history of childhood hearing loss, ototoxic medications, infections)
• Your provider number and contact details for the report

A standard GP letter is sufficient. You can also download our one-page referral form for prompts on the clinical information most useful for accurate assessment
